AGED CARE

Staying In Your Own Home For Longer

The same team that supports our NDIS participants also supports older people who want to keep living at home, in Warwick, across the Southern Downs and further out across regional and rural Queensland and New South Wales. The support covers showering and dressing, meals, the housework, getting to appointments and keeping an eye on how things are tracking, delivered by people who understand life in a country town.

Trilogy Care

We are a service provider partner of Trilogy Care, who hold the approved provider role under the government's Support at Home program. If you self-manage your funding with Trilogy, SD Therapy Supports is one of the providers you can choose.

HOW WE HELP

The Support That Keeps You Home

Every one of these supports happens in your own home, or wherever else you need to be that day.

Help Around The House

Showering, dressing, meals, laundry, cleaning and the shopping, done at the times you would normally do them. Our support workers come to you, and it is the same people week to week so you are not explaining yourself again each visit.

Getting Out And About

Appointments, the shops, church, the club or a visit to family. We drive, we come in and support you if you would like, and we bring you home again. We offer mobility support and the chance to enjoy everything the community has to offer.

Living Well With Dementia

Familiar faces, a steady routine and support that goes at your pace, at home where the surroundings already make sense to you. We keep families in the loop and work alongside your GP so changes get noticed early.

Nursing, Medication And Palliative Care

Medication management, wound care, continence support and chronic condition monitoring from our registered nurses, all delivered at home. Palliative support too, for people who would rather stay somewhere familiar than spend their last months elsewhere. We will tell you early if something needs a doctor.

AGED CARE QUESTIONS

What People Usually Ask First

Do I need to be on the NDIS to get this support?

No. Aged care at home sits outside the NDIS entirely. It is government-subsidised support for older people, arranged through My Aged Care and assessed on what you need day to day. If you are already an NDIS participant, your supports carry on as they are.

Trilogy Care is the approved provider and holds that role with the government. Under their self-managed model you choose the people who actually come to your house, and SD Therapy Supports is one of the providers you can choose. In practice, the funding side sits with Trilogy while the support in your home comes from our local team.

Eligibility for government-funded aged care is decided by My Aged Care, not by us. Start with a call to My Aged Care or a visit to their website, and they will arrange an assessment. Once you know what you have been approved for, come back to us and we will talk about what we can do. If you do not qualify, or you would rather not wait on an assessment, we can also work privately with no government funding involved.

Absolutely. That's the benefit of a self-managed arrangement. You are choosing the provider rather than being allocated one, so the people who come to your house stay the same. If someone is not the right fit, tell us and we will change the roster, no explanation needed.

We would start gently, with short visits at the same time of day and the same worker each time, doing something ordinary rather than personal care straight away. Familiarity does most of the work. Building that trust can take a few weeks, and that is fine. We would rather take the time than force the issue.

Needs change, and usually not on a schedule. Talk to us as soon as you notice things getting harder and we will look at what can be adjusted straight away, and what needs to go back through My Aged Care for review. Someone is on call around the clock in the meantime.
