NDIS TELEHEALTH

Support Coordination Wherever You Live

If your NDIS plan includes support coordination or a psychosocial recovery coach, you can work with us over a video call from your own home or community setting, wherever is most comfortable to you. Even though we're based in Warwick on the Southern Downs, we provide support coordination to participants all across Australia, from north-west New South Wales, all throughout regional and rural areas of Queensland to the other side of the country, with nobody needing to travel.

LEVEL 1, LEVEL 2 AND RECOVERY COACHING

Turning Your NDIS Plan Into Real Support

Your NDIS plan is the starting point, but making it work in everyday life is where we help. We connect you with the right providers, coordinate your supports and keep everything moving in the right direction while ensuring you remain in control of every decision.

Level 1: Support Connection

Help to understand what is in your plan and connect with the providers, community groups and mainstream services that will deliver it, whether that is a physio in Toowoomba or a social group in your own town. Once the supports are running, we step back and check in.

Level 2: Support Coordination

Ongoing help for a plan with several moving parts: support workers, therapists, housing, health services and family all pulling in the same direction. We sort out service agreements, chase the provider who has gone quiet, step in when something breaks down and gather the reports before your plan review.

Psychosocial Recovery Coaching

For participants with a psychosocial disability, a recovery coach does the connecting a support coordinator does and also works with you on your own recovery: building on your strengths, planning for the harder stretches, staying linked with your mental health team and keeping the week going when motivation drops.

Know-How That Stays With You

Part of the job is helping you do more of the coordinating yourself, if you want to. We explain each step as we go, how to book a therapist, what to say when a provider cancels, so that over time you make the calls and we are the backup.

SUPPORT COORDINATION QUESTIONS

How Support Coordination By Video Call Works

How does support coordination work over a video call?

Much the same as it would across a table. We email or text you a Microsoft Teams link, you tap it at the agreed time and we talk, with family, a carer or an existing support worker joining if you want them there. Between calls, the chasing, the paperwork and the follow-up happen at our end.

Yes. Support coordination and recovery coaching happen over a video call, so we deliver them Australia-wide. Whether you are in Moree, Gunnedah or Tamworth, on a station west of Roma, or over in Western Australia or Tasmania, the service is the same.

A support coordinator works with participants with any disability and focuses on putting the supports in your plan into place. A recovery coach does that for people with a psychosocial disability and adds coaching on your own recovery, usually with more time together. Your plan says which one you have funding for, and if you are not sure, send us a copy and we will tell you.

No, and plenty of people have both. A plan manager handles the money side of your plan, paying the invoices and keeping the accounts. A support coordinator handles the people side: who delivers each support, how they start and how they fit together. We provide support coordination and work alongside whoever manages your plan, whether that is a plan manager, the NDIA or you.

Yes. You can change support coordinators at any point in your plan, and a support coordinator who is hard to reach or does not know your town is reason enough. Tell your current provider you are moving, and once their notice period has run we take over, with your consent to contact them so that nothing you have set up gets dropped in the handover.

Call us or fill out our contact form and tell us where you live and what your plan includes. We will ask for a copy of your plan, send you a short service agreement to read over and book the first video call at a time that suits you. From then on you have one person who knows your plan and how to reach everyone in it.
