The Full Range Of Support We Offer

Our supported independent living homes in the Southern Downs are only one part of what SD Therapy does. The bulk of our work is in people's own houses and out in their communities across regional and rural Queensland and New South Wales, delivered by the same small team that knows your needs and preferences. We are also a registered NIISQ provider, so people seriously injured in a motor vehicle accident in Queensland can bring their NIISQ plan to us. Someone is on call around the clock, for the nights when something cannot wait until morning.

If the support you are after doesn't fit any of the categories below, it is worth asking anyway, because a lot of what we do spans several services at once to give our participants the unique outcomes they deserve.

Daily Living & Personal Care

Help with showering, dressing, meals, medication and keeping the house running, at the times of day that suit you.

Short-Term Accommodation & Respite

A short stay in one of our comfortable properties, so you get a change of scenery and your family gets a breather.

Supported Independent Living (SIL)

Day-to-day help in shared accommodation in Warwick, with a familiar team and someone on call all night.

Individualised Living Options (ILO)

A home-and-living arrangement you help design, whether that means living alone, with a housemate you chose, or with a host.

Community Access

Getting out to the people and places you enjoy, with support to plan the trip, travel there and get home again.

Capacity Building

Working on the goals written into your own plan, step by step and at whatever pace you need to make progress.

Employment Support

Support to find work worth having, and to settle in once you have started, at your own pace.

Advocacy & Mediation

Someone beside you in meetings, and a hand sorting out a disagreement between housemates, family members or services.

Registered Nursing & Clinical Support

Registered nurses on our own team for wound care, medication, continence and palliative support, alongside the people who already support you.
